‘Hospitals make mistakes every week’
Hospitals make mistakes during simple operations about once a week – for example by amputating the wrong toe – Gerrit van der Wal, head of the healthcare inspectorate, told ANP. His comments came during the presentation of a report on medical blunders.
The report concludes that a breakdown in communications and inadequate patient files are the most common causes of medical errors.
